A Primary Care Network (PCN) is a community-based network of health care professionals and clinics who plan and deliver the health care needs of a community.

The Surrey-North Delta Primary Care Network (PCN) makes it easier for Family Doctors and Nurse Practitioners to provide the best possible care for their patients. Centering on the needs of patients and their families, our PCN connects primary care providers, health care services and community services ensuring that patients and their families in our community can access the health supports they need. This includes people living in and around:

  • North Delta- West Newton
  • North Surrey-Whalley
  • Guildford
  • Cloverdale-Panorama
  • Fleetwood
  • East Newton
