A Community Health Centres (CHC) provides primary care, prevention and health promotion services. Designed by the community, for the community - every CHC offers different services based on the needs of their community. They connect underserved people of the community who do not have a primary care provider with a family doctor or nurse practitioner. Community Health Centres provide access to preventative and primary health care services to patients who:
- Experience barriers to care
- May have unmet medical and social needs
- May have mental health and substance use challenges
- May require help getting connected to other community services
Other services may include housing, social assistance, cultural supports, English as a second language programs, and food and nutrition support in a culturally safe environment.
