In Burnaby, Fraser Health provides its services within the traditional, ancestral and unceded shared territories of the xʷməθkʷəy̓ əm (Musqueam) [mus-kwee-um], Sḵwx̱wú7mesh (Squamish) [Squ-HO-omeesh], and səlilwətaɬ (Tsleil-Waututh) [slay-wha-tooth] First Nations and home to the North Fraser Métis Association.

Overview

Primary Care Networks (PCN) are community-based networks of health care professionals and clinics who plan and deliver the health care needs of a community.

The Burnaby Primary Care Network team is working with local clinics to help connect people to a primary care provider, either a family doctor or nurse practitioner. This includes people living in the following communities:

  • Burnaby
  • Lougheed
  • Brentwood/Hastings
  • Edmonds
  • Metrotown

If you need a primary care provider, register on the Health Connect Registry to get matched with a family doctor or nurse practitioner in your community.

The Health Connect Registry is the primary tool used by healthcare providers to add patients to their practice. Wait times will vary based on each doctor and nurse practitioner's capacity.
