In Chilliwack, Fraser Health provides its services within the traditional, ancestral and unceded lands of the Stó:lō [staw-low] people, particularly the Aitchelitz [a-che-leetz], Xwchíyò:m (Cheam) [chee-am], Kwaw-kwaw-a-pilt [kwa-kwa-plit], Shxwhá:y [shwhy], Skwah [skwah], Squiala [skwai-all-a], Soowahlie [soo-wall-e], Skowkale [skow-cale], Tzeachten [chee-ack-ten], and Yakweakwioose [ya-kweek-we-oos] First Nations and home of the Chilliwack Métis Association.

Overview

A Primary Care Network (PCN) is a community-based network of health care professionals and clinics who plan and deliver the health care needs of a community.

The Chilliwack, Hope and Agassiz Primary Care Network Primary Care Network (PCN) team is working with local clinics to help connect people to a primary care provider, either a family doctor or nurse practitioner. This includes people living in the following communities:

  • Chilliwack
  • Hope
  • Agassiz
  • Harrison

If you need a primary care provider, register on the Health Connect Registry to get matched with a family doctor or nurse practitioner in your community.

The Health Connect Registry  is the primary tool used by healthcare providers to add patients to their practice. Wait times will vary based on each doctor and nurse practitioner's capacity.
