Overview

A Primary Care Network (PCN) is a community-based network of health care professionals and clinics who plan and deliver the health care needs of a community.

The East Kootenay PCN is working to improve the health of the population through timely access to comprehensive, person-centred, team-based care that is equitable and culturally safe. In the EKPCN, teams of healthcare professionals such as family physicians, nurse practitioners, registered nurses, allied health professionals, clinic staff, midwives and Aboriginal care navigators collaborate to improve delivery of primary care to patients. Find a family doctor or nurse practitioner

The East Kootenay Primary Care Network is working with family practice clinics in East Kootenay communities to help individuals and families find a family doctor or nurse practitioner. If you need a primary care provider, register on the Health Connect Registry to get matched with a family doctor or nurse practitioner in your community.

When you register, you are added to a centralized list for your community. You can also register your family members or a person in your care. Registration takes less than 10 minutes. Be prepared to provide your: 

  • Personal Health Number (PHN) found on your BC Services Card
  • Home address
  • Email address
  • Phone number
