Overview

A Primary Care Network (PCN) is a community-based network of health care professionals and clinics who plan and deliver the health care needs of a community. The Fraser Northwest (FNW) Primary Care Network’s goals are:

  • To create a quality, integrated and coordinated delivery system for primary care that is patient-centred, effective in meeting population and patient needs and delivers a quality service experience for patients 

  • To create the structures necessary to enable all members of the community to receive the primary care they require, by bringing together health authorities, physicians, nurse practitioners, nurses, allied health and other community providers in partnership 

  • To support family physicians who provide longitudinal care through the support of teams, allied health care providers, and easily-accessed health authority services

  • The FNW PCN has implemented the following: contracted Family Physicians, Nurse Practitioners, Registered Nurses in Practice, Indigenous Supports (Family Physicians and Support Worker) and a Primary Care Pharmacist (PCP)

The Fraser Northwest Primary Care Network team is working with local clinics to help connect people to a primary care provider, either a family doctor or nurse practitioner. This includes people living in the following communities:

  • Anmore
  • Belcarra
  • Coquitlam
  • New Westminster
  • Port Coquitlam
  • Port Moody

Find a family doctor or nurse practitioner

If you need a primary care provider, register on the Health Connect Registry to get matched with a family doctor or nurse practitioner in your community. 

Registering using this online tool adds you to a local waitlist for a provider. You may also register your family members or a person in your care. The Health Connect Registry is the primary tool used by healthcare providers to add patients to connect patients without a primary care provider to family physicians or nurse practitioners that are accepting patients. Wait times will vary based on each doctor and nurse practitioner's capacity. 

Patients can also call 8-1-1 to self-register over the phone or register on the provincial Health Connect Registry to get matched with a family doctor or nurse practitioner in your community.
