Last updated

The clinicians and staff of the Mission Division of Family Practice acknowledges that we live and work on the unceded, traditional, and ancestral territories of the Stó:lō People, particularly the Sq’ewlets, Matsqui, Kwantlen, and Leq'á:mel Peoples.

Overview

A Primary Care Network (PCN) is a community-based network of health care professionals and clinics who plan and deliver the health care needs of a community.

The Mission Primary Care Network team is working with local clinics to help connect people to a primary care provider, either a family doctor or nurse practitioner. This includes people living in the following communities:

  • Mission
  • Leq'a:mel First Nation
  • Sq'ewlets First Nation

The Mission Primary Care Network is a collaboration that brings together primary healthcare providers, Allied Health, and community partners to deliver high quality team-based primary health care to the residents of Mission, Leq'a:mel, and Sq'ewlets First Nations.

Find a family doctor or nurse practitioner

If you need a primary care provider, register on the Health Connect Registry to get matched with a family doctor or nurse practitioner in your community.

Mission PCN recognizes that there is a great need for family doctors and nurse practitioners in our community. The Mission Primary Care Network team is working with local clinics to help connect people to a primary care provider, either a family doctor or nurse practitioner. The Health Connect Registry is the primary tool used by healthcare providers to add patients to their practice. Wait times will vary based on each doctor and nurse practitioner's capacity.
