Overview
A Primary Care Network (PCN) is a community-based network of health care professionals and clinics who plan and deliver the health care needs of a community.
Richmond is comprised of three Primary Care Networks (PCNs) and represent a partnership between the Richmond Division of Family Practice and Vancouver Coastal Health. They support Richmond residents in achieving their health goals through collaborative, team-based care, connecting physicians, nurse practitioners, clinicians, and program administrators
The Richmond Primary Care Network team is working with local clinics to help connect people to a primary care provider, either a family doctor or nurse practitioner. This includes people living in the following communities:
Richmond West PCN: Blundell, Thompson, Seafair, Steveston
Richmond City Centre PCN: Richmond City Centre, Bridgeport
Richmond East PCN: Gilmore / Shellmont / East Hamilton, Broadmoor, East & West Cambie

The Health Connect Registry is the primary tool used by healthcare providers to add patients to their practice. You can also register family members or people in your care.
To register, you will need to provide the Personal Health Number (PHN), B.C. home address, and contact information for each person being registered. Wait times will vary based on each doctor and nurse practitioner's capacity.
