The South Delta Primary Care Network provides its services within the traditional, ancestral and unceded shared territories of the xʷməθkʷəy̓ əm (Musqueam) [mus-kwee-um] and sc̓əwaθən məsteyəxʷ (Tsawwassen) [tsawah-sen] First Nations and home to the Surrey-Delta Métis Association.

A Primary Care Network (PCN) is a community-based network of health care professionals and clinics who plan and deliver the health care needs of a community.
The South Delta Primary Care Network (PCN) team is working with local clinics to help connect people to a primary care provider, either a family doctor or nurse practitioner. This includes people living in the following communities:
- Delta
- Tsawwassen
- Ladner
